Please use this form to submit your nomination for the Society of Radiographers' Silver Medal.

Guidelines

• UK-based or overseas nationals who are SoR members, past members, or non-members (who are outstanding contributors to the profession) are eligible to be nominated for the Silver Medal.
• Nominations for the Silver Medal require two named nominators.
• All supporting evidence provided by both nominators should be submitted as part of this form. Evidence should demonstrate achievements relating to the following qualifying criteria (as many as applicable):
  • Promotion of the science and practice of diagnostic imaging and/or radiotherapy and oncology for the benefit of the public
  • Published work
  • Trade union and industrial relations activities at all levels
  • Contributions to the professional development and welfare of colleagues
  • Contributions to educational and/or research developments within diagnostic imaging and/or radiotherapy
  • Advocacy and ambassadorial roles within and outside the profession at national and/or international level
  • Any other appropriate contributions
